Please complete and submit the following form with the required information in order to discontinue or withdraw from lessons.

Terms:
Once you have submitted your form, allow for a minimum of three (3) business days for your withdrawal to be processed, after which tuition charges will stop.
​
Tuition that has already been paid will NOT be refunded upon discontinuance of lessons.
If the student is absent for any reason after submission of the discontinue form, student will be immediately removed from our lesson program and no additional lessons will be given.
